The Care Transitions Intervention^SM Video

Thank you for your interest in the Care Transitions Intervention^SM video. The Care Transitions Intervention^SM video is a training and dissemination tool designed for use by multiple audiences, including health care practitioners, health care organizations, policymakers, and patients/caregivers. The objective of the video is to provide a window into the Care Transitions Intervention^SM and the science behind it. The video depicts the transition coach and patient negotiating “real life” transitional care issues and scenarios in a number of settings to illustrate the key facets of the intervention. The video is supported by other tools which can be found on our website: the Protocol Manual, Personal Health Record, Medication Discrepancy Tool, Care Transitions Measure, and Discharge Preparation Checklist.

This video is available in DVD or VHS format. There is no charge; however, in order to send you the video and to track its use for our funders we ask that you complete the fields below. Following submission and processing of this information, we will mail you the video in DVD or VHS format along with our Protocol Manual.

Unfortunately, due to cost restrictions, we are currently unable to send copies of the video to locations outside the U.S or Canada.
